Form 5472 Part V: transactions of a foreign-owned U.S. DE
Short answer
Part V is the Part for a foreign-owned U.S. DE. The instructions say to check its box if the entity had any other transaction not already entered in Part IV, and to describe those transactions on an attached statement.

You must check the box in Part V if you are a foreign-owned DE that had any other transaction, as defined by Regulations section 1.482-1(i)(7) not already entered in Part IV. These transactions include amounts paid or received in connection with the formation, dissolution, acquisition, and disposition of the entity, including contributions to, and distributions from, the entity. Describe these on an attached statement.
Enter the total value in U.S. dollars of all foreign related party transactions reported in Parts IV and VI (and if the reporting corporation is a foreign-owned U.S. DE, Part V) of this Form 5472.

The instructions name formation, dissolution, acquisition and disposition of the entity, and they name contributions to, and distributions from, the entity. They refer to Regulations section 1.482-1(i)(7) for what counts as a transaction.

A hypothetical, only to show the vocabulary
An owner paying an LLC's formation costs out of their own pocket is the kind of event the phrase amounts paid or received in connection with the formation of the entity points at. Whether a specific payment falls within the wording is a question for a qualified professional.
